Lost Student Completion Card

How do I replace my lost Student Completion Card?

If you have lost your completion card, please contact the site where you originally obtained it. If the site no longer exists, or if you do not know how to contact them, please contact your State Coordinator.

If you still cannot find a replacement, please contact us. We recommend you consider retaking the course if it has been more than 6 years.
