For Motorcycle Replacement Cards: Contact the Host Installation Safety Office that issued the original card. Cards issued more than six years ago can not be replaced - attending another training course is recommended. If you completed training through a civilian training provider, please contact that site for a replacement card. The Air Force Safety Center does not maintain copies of course completion records.
