Q. I lost my completion card. How do I get a replacement?
A. The national MSF office does not keep individual student records. If you need a replacement completion card, you need to contact the site at which you took the course.
Q. I lost my completion card, and the site at which I took the course is no longer operating (or the military base at which I took the course is closed). Now who do I contact?
A. In this instance, you need to contact the state program administrator for the state in which you took the course or contact the military program administrator for the branch of service where you took the course.
